Care Coordinator – Co-Occurring Disorders Program
Hope House Addiction Services is opening a new program for individuals with co-occurring mental health and substance use disorders. As part of this new program, we are seeking a care coordinator to assist with complex medical, emotional, and psychosocial needs of this growing population of people in recovery.

Hope House Mission Statement: Our mission is to provide individualized treatment to those living with a substance use disorder (SUD). Individuals will receive education and support allowing them to pursue their life interests with families, employers, and be self-sufficient members of their communities. In addition, we strive to provide current information on the disease of addiction to families and the community at large.
Primary Responsibilities
In conjunction with Co-Occurring clinicians and clients’ individualized treatment plans, the care coordinator will do the following:
Identify resources that meet clients’ individualized needs and facilitate connections between clients and these resources
Act as patient navigator to prevent gaps in care and help clients put treatment plans into action
Rapid triage and crisis intervention
Urgent follow-up for critical psychiatric and medical needs
Assist with insurance coverage, scheduling appointments, transportation, and parts of the treatment plan that client and clinician cannot handle on their own
Manage transitions in care and interagency referrals to other levels of care
Ideal for complex clients seeing multiple providers in multiple settings
Develop and maintain partnerships with outside agencies that offer relevant services to Hope House clients
Maintain active database of community resources and update on a regular basis
Assist clients in scheduling and transportation to outside appointments on a case-by-case basis
Serve as the liaison between Hope House and outside agencies to develop a network of resources appropriate for client needs
Remain in close communication with clinicians and COE staff to ensure client needs are met and consistent with individualized treatment goals
Assist in aftercare planning
Knowledge and Skill Requirements
Existing knowledge of, and/or willingness to learn about, community programs relevant to the needs of clients with co-occurring mental health and substance use disorders
Strong interpersonal skills
Bachelor's level education and/or professional license in Addictions Counseling (ACEP, LADCII, LADCI)
Excellent time management and workflow prioritization
Proficiency with MS Office suite
Self-starter who can work independently
Compassionate and flexible approach to client care
Ability to work effectively as part of an interdisciplinary team
Creative and innovative thinker
Bilingual/bicultural a plus
Demonstrated ability to work successfully in multi-cultural settings with diverse client populations.
